On Demand Grand Rounds: What Most Clinicians Need to Know about Borderline Personality Disorder in 2021

Borderline personality disorder (BPD) is a prevalent, disabling, and in some cases fatal disorder associated with high costs to patients, families, and communities, including healthcare systems. With advances in research and treatment development, prior notions that those with BPD are treatment resistant or untreatable have been revised to an understanding that symptomatic remission occurs commonly and there are many treatments that are effective. However, these treatments are often not accessible or suitable for many patients with BPD or for clinicians in their practice frameworks. Understanding common factors within these treatments can guide the majority of clinicians to deliver informed basic care, even in brief and acute settings. This presentation will identify common factors across effective treatments and will describe Good Psychiatric Management (GPM) as one generalist approach that packages these common principles in terms that most clinicians can incorporate into their practice. Findings on what works for whom, when, and where will be presented to illustrate new directions in research that will help us build more scalable treatment options in steps of care that may be more adaptive to personalize treatment for those with this treatable but potentially disabling disorder.

About the speaker

Lois W. Choi-Kain, MEd, MD, is the director of the Gunderson Personality Disorders Institute. The institute provides training and supervision for numerous proven treatments, including mentalization-based treatment (MBT), dialectical behavioral therapy for post-traumatic stress disorder (DBT-PTSD), transference focused psychotherapy (TFP), and general psychiatric management (GPM). She works nationally and internationally to expand teaching efforts on borderline personality disorder and its evidence-based treatments and engages in research to study resources for training clinicians who need direction and patients who need access to informed care. With her mentor, John Gunderson, Dr. Choi-Kain developed a training program for GPM and has been expanding its applications. Dr. Choi-Kain’s current research focuses on how internet-based tools can increase the scalability of care for BPD, as well as identifying what works for whom in BPD treatment. An overarching goal is to move toward more personalized care without sacrificing scalability.
