Cognitive behavioral therapy was first applied to psychotic disorders by Aaron Beck in 1952, however the first clinical trial of cognitive behavioral therapy for psychosis (CBT-P) was not completed until 1992. CBT-P has been shown to reduce distress related to psychotic symptoms by up to 40% and improve overall functioning. The evidence supporting its use is so robust that it is recommended in the American Psychiatric Association (APA) guidelines as well as the National Institutes of Health and Clinical Excellence (NICE) guidelines for treatment of schizophrenia. However, this treatment remains underutilized and often unavailable for people in the United States experiencing psychosis due to lack of experienced practitioners and lack of understanding regarding the benefit of CBT-P in the treatment of psychotic disorders. This presentation aims to improve understanding of the evidence base surrounding CBT-P in the treatment of people experiencing psychosis and to describe the process of CBT-P.

About the speakers

Dr. Machal joined the department of psychiatry at the University of Nebraska Medical Center/Nebraska Medicine in July 2020 as Co-Director of the Active Support for Psychosis in Recovery (ASPIRE) Clinic – a multi-disciplinary clinic providing wrap-around physical and mental health care to those in the Omaha area living with psychosis. She primarily treats teens experiencing a first episode of psychosis utilizing a combination of evidence-based psychopharmacology, Cognitive Behavioral Therapy for Psychosis, and Behavioral Family Therapy. Dr. Machal is an advocate for the physical and mental well-being of patients through her role as membership chair of the Nebraska Psychiatric Society. She lives in Omaha with her two dogs. Her favorite activities include spending time with her niece and nephew, reading, traveling, and knitting.

Dr. Sarah Fischer is a clinical psychologist that works with the ASPIRE clinic and is the director of the Psychotherapy Training program for the ASPIRE team. Her clinical and research training has focused on psychosis and schizophrenia spectrum disorders, with an emphasis in psychosocial rehabilitation and recovery. She is passionate about ensuring that individuals that have experienced psychosis or have a serious mental illness can receive the best, most comprehensive care available, as well as creating opportunities for different health disciplines to come together to provide this care.
