Affect and impulse dysregulation are characteristics of multiple psychiatric disorders, most notably disorders of mood and personality. The individuals affected are often at-risk populations, have suffered physical and emotional trauma, and present to settings that are under-resourced (e.g., educational, residential, correctional). Our understanding of the underlying neurobiology is rapidly evolving, but our ability to treat disorders of affect and impulsivity pharmacologically is still limited at best. Psychotherapy is a critical tool, and often underutilized due to limited resources. The evidence for effective psychotherapy in these populations suggests incorporation of cognitive behavioral therapy, motivational interviewing, and trauma-informed care into a skills-based program that is manualized and sustainable.  Attendees of this conference will learn about the development, implementation, international dissemination, adaptations, and effectiveness of one such integrated therapy, named START NOW.

This activity is intended for psychologists, social workers, counselors, and other mental health professionals.

Learning Objectives

After this lecture attendees will be able to:

1. Define the skills-based, manualized CBT therapy, START NOW, and its original application in correctional and forensic populations.
2. Discuss the elements of START NOW.
3. Examine the adaptation of this skills-based psychotherapy to diverse patient populations, and the impact on affect regulation and impulse control.

About The Speaker

Robert Trestman, PhD, MD, is Professor and Chair of the Carilion Clinic and Virginia Tech Carilion School of Medicine Department of Psychiatry and Behavioral Medicine. He received his Ph.D. in Psychology and M.D. from the University of Tennessee and trained in psychiatry and neurobiology at the Mt. Sinai School of Medicine in New York City. Dr. Trestman came to Carilion from The University of Connecticut Health Center, where he was a tenured professor of Medicine and Psychiatry. Dr. Trestman has studied the neurobiology and treatment of people with severe mood and personality disorders and conducts translational research on mental health. He has authored over 180 peer reviewed articles and other publications and is the senior editor of the first textbook in the field of correctional psychiatry, the Oxford Textbook of Correctional Psychiatry. Dr. Trestman is the co-developer of START NOW, a skills-based psychotherapy in use in five countries. Dr. Trestman has given over 150 regional, national, and international invited presentations. Dr. Trestman serves as Chair of the American Psychiatric Association Council on Healthcare Systems & Financing, is Chair of the American Hospital Association’s Behavioral Health Council, serves as Chair of the Research Committee of the American Academy of Psychiatry and the Law Institute for Education and Research, and chairs the American Association of Chairs of Departments of Psychiatry’s Clinical Enterprise Committee.
