The increased focus on social determinants of health by the WHO and the CDC presented a springboard for psychiatry to emphasize the social determinants of mental health (SDOMH), which has taken on new urgency during the current national public health and child and adolescent mental health crisis. During the pandemic the impact of socially driven health disparities and inequities were more fully recognized. Individual clinicians, academic departments, and healthcare organizations are discerning strategies to dismantle health inequities while simultaneously addressing the broader mental health crisis. Knowledge on SDOMH can be employed to improve the health of the child mental health, reduce the risks for psychiatric illness and advance clinical care models to optimize treatment response and quality of life for individual patients. While prevention necessarily involves policy changes and public sector interventions, there is a pressing need to clinically address SDOMHs. Trainees and clinicians need to learn how to assess patient exposures to SDOMH and understand the influence of these determinants on clinical presentations and implications for prevention and treatments. The largest impact for addressing SDOMH in child psychiatry will be achieved by incorporating social justice and anti-racist approaches. This presentation will include helpful findings from research and implications for effectively integrating a response to SDOMH into clinical practice, including medical coding, reimbursement and other sustainability strategies, and re-conceptualization of psychosocial-cultural assessments.

  1. Identify at least three strategies for addressing SODMH in child and adolescent mental health services.
  2. Apply SODMH and culture assessment, and anti-racist approaches into treatment planning and delivery.
  3. Plan how to operationalize and sustain a systematic approach to addressing SODMH in clinical practice.

About the speaker

Lisa Fortuna, MD, MPH is an adult and child, and adolescent psychiatrist, Professor and Vice Chair at University of California San Francisco, Department of Psychiatry and Behavioral Sciences and Chief of Psychiatry at Zuckerberg San Francisco General Hospital where she leads the public psychiatry teaching, clinical and research mission. She is an alum of the Harvard Pediatric Health services Research Fellowship at MGH. She has been an investigator on several National Institutes of Health and foundation-funded studies of Latino and immigrant mental health, integrated care and equity in mental health care. Her NIDA funded K23 award focused on the development and implementation of an integrated intervention for adolescents with co-occurring PTSD and substance use disorders conducted in community mental health settings. She adapted and implemented the resulting model of treatment for adult Latin American immigrants as part of a NIDA funded international (US and Spain) multisite randomized clinical trial. Her previous work includes serving as a PI of the Transforming and Expanding Access to Mental Health in Urban Pediatrics (TEAM UP for Children): a 4-year, $10 million project aimed at increasing access to mental health care for children and engage families and testing a behavioral health integration model. She was also PI on a NIMH funded R01 study examining factors for optimizing a model of family navigation for child behavioral health services. She is PI of a PCORI funded pragmatic clinical trial, Kids FACE FEARS, testing two modalities (face-to-face synchronous vs. online asynchronous) delivery of CBT for anxiety in English and Spanish for children and their families. She has authored several publications and presentations on children’s mental health services and featured in news and media on these topics. She has worked with the ACLU and other organizations in preparing Amicus Briefs and other expert testimony on immigrant child mental health and separations.
