The American Psychiatric Association’s Principles of Medical Ethics emphasize competence, respect and up-to-date knowledge as a basis for appropriate professional behavior toward lesbian, gay, bisexual, transgender and queer (LGBTQ) patients. After providing some definitions of helpful terms, this presentation then reviews historical psychiatric attitudes towards LGBTQ patients that could be construed, at best, as patronizing and, at worst, overtly hostile. In modern clinical practice, as opposed to trying to “cure” homosexuality or “transsexualism,” LGBTQ patients are helped to live their lives according to their own natures and desires. This presentation outlines some common clinical questions raised by LGBTQ patients—what is known and not known about the origins of homosexuality and transgender expression, sexual orientation conversion efforts (SOCE), therapist self-disclosure, how therapists should address LGBTQ patients, and controversies surrounding treatment of transgender children—as well as ethical issues raised in these clinical encounters.

Burnout is a term we have all heard, used, and experienced. A less frequent concept is secondary traumatic stress or vicarious trauma, on the spectrum with burnout and compassion fatigue. This time, we focus on the clinicians, the providers, and not directly our patients. The goal of this presentation is to discuss what those are, how to identify them, recognize how they may play out in the legal arena, and how to address them when we fall anywhere on that spectrum.

Suicide is a leading cause of death among young people in the US and the rate of death by suicide in the overall US population has been increasing, unlike in most other places in the world. Suicide affects all age groups and ethnicities and is a source of pain and suffering. throughout our society. An understanding of strategies that can reduce suicide is essential in order to tackle this issue. The presentation will review the current evidence about suicide presentation strategies which have been shown to be effective. These include approaches at the societal level as well as those that may be implemented by individual clinicians.

Despite decades of effort, racial and ethnic disparities in healthcare remain a persistent challenge. This interactive workshop will explore the root causes of this stagnation, with a particular focus on the often-unacknowledged resistance to change that hinders progress towards health equity. Through examination of the ongoing disparities in healthcare outcomes, we will identify the individual and organizational behaviors that obstruct equitable practices. Participants will learn to recognize these "elephants in the room" within their own institutions and develop strategies to dismantle these barriers, fostering a more inclusive and equitable healthcare environment.

Formulation is essential to every treatment. Without a formulation, we are unable to make a diagnosis, collaboratively create a treatment plan, or engage in treatment. Yet formulation is not always taught, and, when it is, this teaching is not always clear. In this talk, Deborah Cabaniss will discuss the work that she and colleagues in the Psychodynamic Formulation Collective have done to operationalize psychodynamic formulation and to place the effects of culture and society on par with other psychodynamic models of the mind. Clinically relevant to all patient settings, this talk will be of interest to trainees and seasoned clinicians alike.
